It appears he traveled to the infamous border town to get cosmetic treatment and for a prosthetic implant, these procedures require general anesthesia and an operating room, which is why he wasn't able to get the surgery done in the Sinaloa mountains where he usually hides from the authorities, this according to what Carlos Loret de Mola wrote on his column Historias de Reportero published on El Universal.

Even El Chapo knows that Tijuana has the best plastic surgeons in the world.
